The Disability Benefits Questionaires (DBQs) are forms used by VA doctors in assessing service-connection and disability compensation and/or pension claims. While they are generally used by VA doctors in a veteran claim, veterans can use their own doctors to fill out these forms in support of their claims.  If your doctor is willing to fill out the DBQ for a relevant disability, it can save time; and perhaps eliminate the potental prejudice a VA doctor may have against a claim.  Click here for a link to all of the forms.

If the VA refuses to accept a DBQ filled out by your doctor, contact my office, and I can assist in responding to the VA's request that one of its own doctors conduct a medical examination.
